Escasez de factores influyentes en la evolución del shock cardiogénico isquémico.

Resumen

Objetivo: Detección de variables con influencia sobre el pronóstico de pacientes con shock cardiogénico de origen isquémico durante el seguimiento

Métodos: Estudio observacional prospectivo de los pacientes que ingresaron en la Unidad Coronaria del Hospital General Universitario de Albacete entre el 1 de enero de 2006 y el 1 de enero de 2011 con diagnóstico de shock cardiogénico de origen isquémico que fueron tratados mediante revascularización coronaria en las primeras 72 horas del desarrollo del cuadro. Se realizó un seguimiento de los pacientes durante el ingreso hospitalario y tras el alta, completando un seguimiento de 5 años. 

Resultados: Ingresaron un total de 97 pacientes con diagnóstico de shock cardiogénico de origen isquémico, de los cuales 44 fueron pacientes de edad igual o mayor de 75 años. El porcentaje de varones en el grupo de pacientes ? 75 años fue de 56%, cifra significativamente menor respecto al número de varones del grupo de pacientes < 75 años (81%, p: 0,009). No se encontraron diferencias significativas entre ambos grupos de edad en cuanto a los antecedentes de hipertensión arterial, dislipemia, enfermedad aterosclerótica previa o patologías asociadas (EPOC, ACV o insuficiencia renal crónica). Sí hubo un mayor número de pacientes diabéticos (20,4% vs 49%, p: 0,004) y menor número de fumadores (4,5% vs 39,6%, p: 0,001) en el grupo de pacientes ? 75 años. Tampoco se encontraron diferencias en cuanto al tipo de infarto de miocardio (IAMSEST vs IAMEST) ni en la localización del mismo entre ambos grupos, siendo similar la incidencia de enfermedad de tronco coronario izquierdo o enfermedad multivaso. Se realizó revascularización coronaria percutánea en todos los pacientes sin observarse diferencia entre ambos grupos. No hubo diferencia en cuanto a mortalidad durante el ingreso hospitalario según el género de los pacientes pero sí se observó una clara influencia de la fracción de eyección ventricular izquierda en el pronóstico. De los pacientes fallecidos durante el seguimiento, el 73,8% presentó una FEVI < 35%, p: 0,02). Al realizar un análisis de supervivencia por grupos de edad estratificado por FEVI, de nuevo se demuestra que con independencia de la FEVI, los pacientes con edad ? 75 años tienen una menor supervivencia frente a los pacientes más jóvenes.

Conclusion: Las únicas variables asociadas de forma independiente con mortalidad durante el seguimiento fueron la edad ? 75 años, la FEVI < 35% y la enfermedad aterosclerótica previa.
